Survival after a diagnosis of AJCC Stage IV prostate cancer ? THE ...

For many men, a diagnosis of prostate cancer of any stage or grade ? even of the lowest level of risk ? is hard to deal with. If they have progressive disease after first-line therapy, it may appear to be an overwhelming issue.

This morning, on Ask Arthur, we received a pleasant surprise, which may offer great hope and a ?real life? perspective for the many men who do, indeed, find that first-line therapy has failed to eliminate their cancer, and that they have a rising PSA ?

We will let you read the actual tale for itself. However, for those of you who haven?t been dealing with prostate cancer since the mid to late 1990s, there was an earlier time ? and an earlier version of this web service. About the only thing in common between the two was ?Ask Arthur.? Then, as today, Arthur sought to help individual men with questions about their diagnosis and treatment. Today we also provide a much wider service too ? through the use of our social network, and the calls on Arthur?s time and expertise are far fewer than they were back in 1995 to early 1998, when the original InfoLink site was one of few places you could get to if you entered ?prostate cancer? into the relatively primitive search engines of that time.

The point to all this, however, is that progressive prostate cancer is far from being the end of life. The gentleman who left the message for Arthur last night was initially diagnosed in his mid to late 40s. By 1997 he had a rising PSA, had been told he had Stage IV disease, and that he had 2 to 3 years left to live. But not at all.

Prostate cancer is an unusual disease, and we still know little about the biological drivers of prostate cancer in specific individuals. What we do know, however, is that most men diagnosed with prostate cancer today are not going to die from this disease if it is appropriately managed; that many men diagnosed even with late-stage, progressive disease, may live for a decade or more after their diagnosis; and that we are at the beginnings of a revolution in the management of progressive and advanced forms of prostate cancer, as may be discovered through innovative trials like this one, that has just started to recruit men with relatively early stage metastatic, castration-resistant prostate cancer to bone only.

It is also worth noting part of Arthur?s response to his correspondent:

Arthur believes that the cultural and social biases that kick in (among patients themselves, among family members, and among the professional community) when any diagnosis of cancer is given often still reflect the mindsets of the 1950s as opposed to the 21st Century.

?Cancer? is a loaded word. We do indeed often respond to it in ways that demonstrate our misconceptions about the implications of such a diagnosis. Arthur wasn?t a young man even back in the mid-1990s, but his brain still seems to be working pretty well today!

Of course this is hardly a unique tale. There are many other men today who have been living well with advanced forms of prostate cancer for a decade and more. We need to keep building on that realization ? whether we are patients, healthcare professionals, family members of patients, or just educated consumers. Reacting with fear to an initial diagnosis of prostate cancer is still very ?normal? today. Hopefully, by about 2020, maybe we will have managed to demonstrate that such a reaction is no longer sensible because the management options are so much better than they were back in 1995-7 when Arthur first started to offer men his on-line assistance.

Is Egypt about to Elect an Islamist President?

Registering to run for president of Egypt will begin March 10. The military moved it up from April 15 to show that it is handing over power to the civilians. As I?ve said before, I?ve never seen any evidence that the army is not going to turn over control of the country to a new, elected president. All of the mass media and political hysteria to the contrary, the generals don?t want to hold onto the government.

Has the Brotherhood?s success in parliamentary elections gone to its head? Has the weak international response to its ascendancy emboldened the Islamists to seek total power now rather than go slow and be patient? It?s starting to look that way.

The Muslim Brotherhood has announced once again that it will not run a candidate for president in the elections projected for June. ?The Muslim Brotherhood will not support Abdel-Moneim Abul-Fotouh or any candidate,? says Muhammad al-Badi, the leader of the Brotherhood.

But this is misdirection. The Brotherhood?s influential spiritual advisor Yusuf al-Qaradawi is supporting Abul-Fotouh. And guess what? The Brotherhood is going to support Abul-Fotouh ?unofficially.? How? Simple, through the ?independent? Justice and Development party supporting an ?independent? presidential candidate. Brotherhood leader Muhammad al-Badi now says that the president must have an ?Islamic background? and by that he rules out any ?secular? candidate.

Egyptian voters who backed the Brotherhood?giving it 235 seats, 47 percent of those in parliament?will vote for someone. The Brotherhood doesn?t own their votes but presumably most of them will support an unofficial Brotherhood candidate.
The Salafists, with 121 seats, almost 25 percent of parliament, will probably have their own candidate.

While this seemed impossible last year it is now conceivable that the two leading presidential candidates will be Islamists and thus Egypt will have an Islamist president. That would mean the timetable for turning the country into an Islamist Sharia state could be vastly accelerated. It?s up to the Brotherhood to decide whether to move cautiously toward state power or floor the accelerator.

If the reported plan for the election is accurate, the rules drawn up by the military help the Islamists. To run for president requires endorsement by 30 members of parliament. Only four parties have that many?the Brotherhood?s front group, the Salafists, the Wafd, and the Egyptian Bloc (Free Egyptians Party). Can individual Brotherhood members endorse a candidate without facing party discipline? Again, since the Brotherhood?s party is nominally independent of the Brotherhood, al-Badi?s statement does not restrict its freedom of decision.

According to the Egyptian media, each party can nominate one candidate. While some among? the 80 members of small parties or independents could band together in some combination to nominate someone, the maximum number of candidates would be restricted to five, probably less.

New Haven, Conn. ? Yale scientists have discovered the molecular pathway by which maternally inherited deafness appears to occur: Mitochondrial DNA mutations trigger a signaling cascade, resulting in programmed cell death. The study is in the Feb. 17 issue of Cell.

Mitochondria are cellular structures that function as "cellular power plants" because they generate most of the cell's supply of energy. They contain DNA inherited from one's mother. Mitochondria determine whether a cell lives or dies via the process of programmed cell death, or apoptosis.

The Yale scientists focused on a specific mitochondrial DNA mutation that causes maternally inherited deafness. The mutation occurs in a gene that codes for RNA in mitochondrial ribosomes, which generate proteins required for cellular respiration. The team found that cell lines containing this mutation are prone to cell death not directly due to the mutation, but rather because it enhanced a normal chemical modification of the RNA called methylation, which regulates ribosome assembly.

"Our lab had previously discovered that overexpression of the enzyme responsible for this methylation could cause cell death, even in cells without the deafness mutation," said corresponding author Gerald S. Shadel, professor of pathology and genetics at Yale School of Medicine. "But when the researchers overexpressed the enzyme in mice to mimic the effects of the mutation," he said, "we were astonished to discover that the animals progressively lost their hearing, reflecting how such disease would develop due to a known pathogenic human mitochondrial DNA mutation. This new mouse model will be instrumental in understanding genetic and environmental factors known to impact mitochondrial disease pathology."

The researchers found that reactive oxygen molecules produced by diseased mitochondria are what trigger events leading to a cell death-inducing gene expression program. By genetically depleting the protein ultimately responsible for activating this programmed cell death response, they were able to restore normal hearing to the mice.

The study not only sheds light on inherited deafness in humans, but possibly also age-related hearing loss and other human diseases. First author Nuno Raimundo, a postdoctoral associate in pathology, said, "Mitochondrial diseases are complicated because different tissues are affected in often unpredictable ways. Defining the molecular mechanism underlying death of only a specific subset of cells in the inner ear is a major step toward unraveling this complexity."

US man tries to evict 98-year-old mom on birthday

FAIRFIELD, Connecticut (AP) ? A woman whose son served her with eviction papers on her 98th birthday is fighting his efforts to remove her from her home.

Mary Kantorowski's oldest son, 71-year-old Peter Kantorowski, says he's concerned about her well-being. When he last saw her eight months ago, she seemed disoriented and was living in poor conditions, he said.

Peter Kantorowski, who owns the house, says his mother has rejected his suggestions to live with him or in a nursing home. A trial is set for March 2.

"I didn't think he would do it," Mary Kantorowski said. "My husband worked hard, difficult jobs to buy this house. He built the garage and did a lot of work on the house and he told me never to leave it."

Her husband, John, died in 1997.

"This is just a despicable situation," her lawyer, Richard Bortolot Jr., told the Connecticut Post newspaper. "Mary has been living here happily paying all the expenses for the house and now her son, Peter, comes along and is telling her, 'Get the hell out,' so he can sell it."

Mary Kantorowski's other son, Jack Kantorowski, told WTNH-TV: "There are no other words to call him. He's just a scumbag."

Bortolot said a probate court stopped Peter Kantorowski from trying to sell the house after the eviction papers were served in December.

Peter Kantorowski says he's just looking out for his mother's best interests.

"I'm not throwing her on the street," he said. "At her age, at 98, I'm sure that she should be with people of her peers. She should have her meals on time."

Meanwhile the Japanese auto giant, Mitsubishi India, has decided to introduce the newly designed Pajero sport in the Indian car market by the end of 1st quarter which is just a month and a half away.

It is expected that the new Mitsubishi Pajero sport has the potential which will help the company to ramp up its market position in Indian car market. Despite being a high end sporty vehicle Pajero did not manage to grab huge market sales. Earlier Pajero was launched through CBU route which made it costlier. Now Mitsubishi has decided to assemble Pajero sport locally at the Hindustan Motor?s assembly line, which allows them to price Pajero sport competitively in India. This step provides the new Mitsubishi Pajero sport an edge over its prominent competitors in India like the Honda CR-V and Renault Koleos which are CBU?s because of which the price tag is almost double than Pajero.

The noticeable change or features of the upcoming SUV Pajero sport which is available in global market is a massive turbocharged diesel engine churning out 135 PS of maximum output power and amazing torque value of 320 Nm. The diesel engine of the global SUV is linked with five speed manual/automatic transmission gearbox. It would be quite interesting to see if the Japanese auto major brings in the automatic version of Pajero in the Indian car market. Recently its biggest rival Toyota India reintroduced a modified version of Toyota Fortuner with automatic transmission at the Auto Expo 2012. The company is required to re-schedule its plans for Indian market because the Pajero SUV has a higher demand in the country when compared to the Mitsubishi Outlander. Additionally, the Japanese auto biggie needs to improve the facilities and the standard of the service centers, as these are some of the major issues which turn out to be a major factor which cater buyers.
